Indicator for redundant blocking
The blocking categories on OpenDNS are changing all the time. So I'm sure people have set personal access list entries that block domains that are later blocked by a category they use, so the domain is blocked redundantly. The same thing could happen if someone specifically permitted a domain that was later cleared from a blocklist category and no longer required on their personal access list.
It would be nice if there was a way to see this information on the access list screen. Maybe you could annotate the access list entries with an asterisk out to the right and something like, " * These domains already blocked by <category>" at the bottom of the list?

Just a comment:
If you attempt to add a domain to the blacklist/whitelist, and it is categorized, it asks you already if you want to add just this single domain, or if you want to block/unblock one of its categories instead. If it is not categorized, it will not give you this option. A good starting point, but still something to do in view of later changes of domains related to categories, as you pointed out.
Also, I think this idea was raised before, but may be a long time ago...
