Log which device requested each domain
Obviously, in a family situation, the criteria for parents, older kids, and younger kids would logically be different. It would be very helpful to know which devices are going to which sites. For example, for us, if my 22 year old went to some sexually oriented site, frankly, I wouldn't care, but if my 12 year old did, I'd be alerted that we need to talk.

This same basic request has been made many times, and explained many times in multiple threads.
In short, if you are using the free OpenDNS service, due to the way that DNS works, it is not possible to determine which device on a local network made a DNS request because that information is not transmitted to OpenDNS or any other DNS provider.
There are several pay services that allow individual devices to be logged, generally by installing a piece of agent software on the device or have a virtual appliance somewhere in the network (but that option is generally only practical for business networks).
Umbrella Prosumer might fit your needs. You should contact either OpenDNS to discuss it, or one of OpenDNS partners, who can also provide a deeper level of support and monitoring not only for OpenDNS but offer related services as well.
