"Is it possible another computer on network installed utility and is able to enable/disable controls?"

This could well be the case...

    Are there other scenarios?

Don't want to peg him incorrectly w/ blame, but I cannot understand how this happens.

My understanding from previous questions is that your servers base enable / disable based on MAC of Netgear router, not IP addr.

As well, one thing I see lacking in the netgear.opendns.com is an indicator that says the whether the controls are enabled for the router with MAC address.

A broken router?  Or a router with a bug in the firmware?  Refer to Negear support.

No, my servers are unrelated to Netgear routers.  I'm a user like you.  But yes, LPC is based on the device ID, not the IP address as with normal OpenDNS.
