"if I have these image sites blocked (google/images, bing/images)"

How exactly did you do this?

"why does the porn pictures still appear?"

Because you apparently don't have blocked them, or your settings do not work due to a misconfiguration or so.

Right-click the offending image to see from the properties what domain name it is hosted on, and block that domain's related category or the domain name individually.

If it still does not work, post the complete plain text output of the following commands here:

    nslookup -type=txt debug.opendns.com.

    nslookup www.exampleadultsite.com.

OpenDNS cannot block search engine results without blocking the search engine itself. This is because the results are hosted on the search engine domain. OpenDNS can, however, block access to the links in search results.

For example, if 'Playboy' is searched in Google then clicking the link to playboy.com would result in a block page if adult content is blocked on your network.

You will have to block all of google.com or bing.com to block the images that appear in their search results.  You can block explicit.bing.com to help prevent explicit search results but Google hosts their results explicit and not on the same domain.
